Media Release Agreement — Founding Patient Program

1. What We’re Collecting

As part of the Founding Patient Program, Blue Anchor will document my journey through photos and/or videos, which may include:

• Before and after treatment photos
• Video or photo footage of my trip (airport arrival, clinic visits, sightseeing, etc.)
• My verbal or written testimonial about my experience

2. Consent to Marketing Use

I consent to Blue Anchor’s use of these photos, videos, and testimonials — including my likeness, voice, and name — for Blue Anchor’s marketing and promotional purposes, including but not limited to:

• TikTok, Instagram, and YouTube
• The Blue Anchor website (askblueanchor.com)
• Other marketing materials (e.g., presentations, ads)

3. Voluntary & Revocable

My participation in this Founding Patient Program is conditioned on this media cooperation, as reflected in my discounted Founding Patient rate. I may withdraw this consent at any time by written request to Blue Anchor for content not yet published. I understand that content already published (e.g., posted to social media) may not be fully retractable, and Blue Anchor will make reasonable efforts to remove it upon request but cannot guarantee removal from platforms or from users who may have already viewed, saved, or shared it.

4. No Additional Compensation

I understand that my Founding Patient discount is the sole consideration for this media cooperation, and no further compensation, royalty, or payment will be made for the use of this content.

5. Governing Law

This agreement is accepted electronically when the Client checks the corresponding agreement box on Blue Anchor’s application form. No separate physical or digital signature is required.
